• Download the Firma Medical Prescription Form.
• Take this form with you to your appointment or have your physician
  complete the appropriate section.
• Fax or mail the completed form back to Firma Medical.

Clinical Support
Once we receive all the necessary paperwork, a VED Vacuum Pump will be mailed directly to you. Once you receive the pump a clinical specialist will contact you. At Firma Medical patients talk to patients. We have a team of patients who conduct our telephone consultations.

Reimbursement Support
